Filing a Claim for Hospitalization Expenses
For policies with Hospital Income Benefit (HIB)
A Hospital Income Benefit (HIB) rider attached to your policy provides you with a cash benefit for each day of hospital confinement up to a certain number of days specified in your contract.
Please refer to page 3 of your life insurance policy contract for your schedule of benefits and specific details.
- Duly accomplished claim forms:
- Claimant Statement for Hospitalization / Medical Reimbursement
- Certificate of Attending Physician for Hospitalization / Medical Reimbursement
- Original or Certified Copy of the Statement of Account from the Hospital
- In the absence of the SOA, you may submit a Hospital Certification which should contain the inclusive confinement dates. This certification must be signed by an authorized personnel from the hospital’s Billing or Records Section and NOT by the Attending Physician.
- Police Report, if cause or confinement is due to an accident. If this was not reported, please submit the Insured’s own narration of the incident.
For policies with Hospitalization and Surgical Benefit
A Hospitalization and Surgical rider attached to your policy provides you with room and board, hospital services and surgical benefits.
Please refer to page 3 of your life insurance policy contract for your schedule of benefits and specific details.
Claim Requirements:
- Duly accomplished claim forms:
- Claimant Statement for Hospitalization / Medical Reimbursement
- Certificate of Attending Physician for Hospitalization / Medical Reimbursement
- Original or Certified Copy of the Statement of Account from the Hospital

- Official Receipts of Hospital Bills
- Official Receipts of Physician’s Professional Fee or Services, if surgery was performed
- Official Receipts of medicines bought outside the hospital within the confinement period
Further Reminders
The processing of the claim begins when the basic claim forms and other related requirements are submitted to any Philam Life Office nationwide. Additional requirements may also be required as the company deem necessary in some instances.
Upon receipt of the required documentation, we will then begin the benefit assessment, evaluate the claim and provide you with commitment dates. You shall be notified of the final decision once available. If the claim is approved, proceeds may be released to the Philam Life office nearest you upon presentation of proper identification.
